Taylor Swift Debuts Star-Studded ‘Patient Zero’ Music Video During the 2026 MTV VMAs

Taylor Swiftused the 2026 MTV Video Music Awards stage to premiere the music video for her latest single, “Patient Zero.”

After accepting the inaugural Artist Director Honors during the Sunday, September 27, award ceremony held at Los Angeles’ Peacock Theater, Swift, 36, premiered the video starring Dakota Johnson, Colin Farrell and Cara Delevingne.

The video began with Swift joining Johnson, 36, in a cemetery before footage flashed back to the Verity actress appearing melancholy over her romance with Farrell, 50. In one scene, the duo stood on opposite side of dining table, reminding Us of Swift's Eras Tour performance of "Tolerate It."

Later, Swift observed Johnson accompanying Farrell to accept a big award, where Delevingne, 34, cameoed as a "lusty other woman," per Swift's tribute to her costars shared via Instagram after the event. After she tripped and fell, Johnson apologized on the way home before the Irish actor condemned her for trying to humiliate him.

"I don't know what happened, I'm sorry," she said, before Farrell noted he thought she was different. She added, "Different from who?"

A flashback scene revealed that Farrell had previously proposed to Swift. In the end, Johnson mourned a "Beloved Wife and Daughter" who lived from 1989 to 2022.

Swift released “Patient Zero,” one of four new songs on the extended version of her Life of a Showgirl album, on Friday, September 25.

“I was hanging out with a bunch of friends, and one of my friends told us that she had been reached out to by the new girlfriend of an ex of hers, asking for guidance on if she had had the same experiences with this guy,” the Grammy winner told the U.K.’s Capital FM of her writing inspiration. “Everyone in our group just immediately started saying like, 'Oh, I've been through that too.’”

Swift continued, “I thought to myself, ‘If this has happened to all of us, it’s such an interesting kind of angle on humanity and on relationships. I wonder if I could write something from the perspective of getting this phone call, and really only from the perspective and the side of the phone call of the ex who’s been through it before.’”

Swift, who married Kansas City Chiefs tight end Travis Kelce in July after three years of dating, has not publicly shared her “Patient Zero” muse. Several fans have theorized that the track might be inspired by Swift’s ex-boyfriend Joe Jonas following his 2024 divorce from her friend Sophie Turner or a potential warning to the singer’s ex Matty Healy’s wife, Gabbriette Bechtel.

In addition to “Patient Zero,” The Life of a Showgirl: The Encore featured songs “Babylon,” “Pink Clouding” and “Cleveland!”, the latter of which is a sweet tribute to Kelce, 36, and named after his Ohio hometown.

As for the “Patient Zero” video treatment, Swift also directed the footage. In honor of her plethora of directorial achievements, the pop star also took home the inaugural Artist Director Honors award on Sunday.

“Across her work, Swift has brought an increasingly expansive approach to storytelling, creating interconnected narratives, cinematic worlds and recurring details that invite audiences deeper into each project,” a press release shared before the ceremony read. “The inaugural MTV VMA Artist Director Honors recognizes that body of work, while also underscoring the growing role artists play in shaping how their music is brought to life on screen.”

Among her 11 nominations this year, Swift was recognized for the direction of her 2025 video “Opalite.”
